How do fentanyl test strips work?

June 4, 2024

Fentanyl test strips are designed to reduce the risk of overdoses from drug use. Anyone can purchase and use fentanyl test strips to test for the presence of fentanyl in a drug. They use the same technology as an at-home pregnancy test and were originally developed to detect the presence of fentanyl in urine. Studies have shown that fentanyl test strips not only decrease overdose deaths, but in many cases prevent drug use altogether. When young people use fentanyl test strips and see for themself that their drug has fentanyl inside, they typically do not consume the drug and instead throw it away.

RELIABILITY

Fentanyl test strips are highly effective at detecting microscopic amounts of fentanyl in drugs. That’s important, because doses of fentanyl as low as 2mg - the size of a few grains of sand - can be lethal.

Like most things, the test strips are not 100% accurate in all cases. The test strips sometimes show a “false positive” when there is no fentanyl. There is little to no documented evidence of a “false negatives” occurring, where the results would incorrectly state that the substance contained no fentanyl when it truly did.

A redesigned type of fentanyl test strip, available from DanceSafe, may be less likely to produce false positives when used according to the instructions.

HOW TO USE TEST STRIPS

  • This fact sheet from the California Department of Public Health explains how the strips can be used effectively. For accurate results, it is critical to follow the instructions precisely.

IMPORTANT REMINDER

  • If someone is using drugs without a fentanyl test strip, they can make sure to have Naloxone on hand and use “one person at a time” to make sure someone is sober and able to respond in case of overdose.

If you're a person who needs opioids and you have a dependence on them, it's very useful to use a fentanyl test strip to determine if there's fentanyl or one of its analogues in the substance you're about to take. If you're in your 20s, 30s, or 40s and you're getting some cocaine, you should know that cocaine has been contaminated, and the frequency of contamination is rising. Fentanyl test strips can be used on any drug, as long as you prepare that drug correctly. So, we instruct users to crush whatever it is in its entirety, mix it up, and then use about 50 milligrams of that powder.

There is a risk of getting a false negative; the test may not pick up the particular analog, and the dilution rate might not be right. So, it's not foolproof because the problem is inconsistent dosing. You can't shave some of a pill and use that as your sample and be 100% sure that there's not fentanyl in the other portion of the pill. And you certainly can't test one pill from a baggie, have it come back negative, and be confident that the rest of the baggie does not contain fentanyl. The main takeaway from that message is: If anyone presents a pill to you and says, "Don't worry, this has been tested," that is not a true statement, and you cannot trust any pill that you get on the street.

While the expectation is that you don't take any pill or powder that's coming from an unregulated source, but if you did, it is non-negotiable to carry Narcan with you. It is non-negotiable to not use alone, and it is non-negotiable to test your drugs. These are the steps you have to take. While it's not perfect, it's the bare minimum to keep yourself alive.
